We provide independent, evidence-based investigations designed to identify root causes, assess risk exposure, and provide defensible findings. Our investigations follow the ICAM (Incident Cause Analysis Method) methodology, using PEEPO (People, Environment, Equipment, Procedures, Organisation) causal analysis to ensure findings are robust, and consistent. We deliver findings you can rely on and act on with confidence.

What We Offer

Incident Review & Investigation

Tier 1 — Standard Investigation

Applies when most or all of the following are true:

  • Injury or incident did not meet WorkSafe's notifiable event threshold (no serious injury/illness, no notifiable near-miss)

  • Single site, single shift/incident window

  • Fewer than 4 witnesses to interview

  • No suspected regulatory, insurance, or legal action anticipated

  • Root cause is likely contained to a single process, control, or piece of equipment

  • Investigation completed within a single site visit (1–2 days on the ground)


Serious Harm / High-Risk Investigation

Tier 2 — Serious Harm / High-Risk Investigation

Applies when any of the following are present:

  • Incident meets or likely meets WorkSafe's notifiable event criteria under HSWA (serious injury/illness, notifiable illness, or a notifiable near-miss with potential for serious harm)

  • WorkSafe has been notified, or notification is anticipated

  • Multiple witnesses (2 or more) or more than one work area/team involved

  • Root cause investigation likely spans more than one system or control failure (e.g. training gap and equipment fault and supervision breakdown)

  • Client anticipates needing the findings for ACC, insurance, or internal disciplinary purposes

  • Multi-day on-site presence required (typically 3–5 days across interviews, evidence gathering, and follow-up)


Complex / Critical Incident Advisory

Tier 3 — Complex / Critical Incident Advisory

Applies when any of the following are present:

  • Fatality, or an incident with reasonable potential to have caused a fatality

  • WorkSafe has commenced, or is highly likely to commence, its own formal investigation or enforcement action

  • Realistic prospect of prosecution against the PCBU and/or an officer personally (s44/s48 exposure)

  • Multiple sites, multiple witness groups, or an extended timeframe of contributing events

  • Client's legal counsel is engaged or expected to be engaged, and findings may need to withstand scrutiny in an ERA, coronial, or court process

  • Ongoing executive/board advisory support is needed beyond the report itself — not just a debrief, but sustained guidance through the incident's aftermath
